Hi! I am sorry to hear about your falls! There are many causes of unexplained fall. One is that the proprioceptive receptors of foot are at fault and they are not able to take in the variation in the floor. This can happen if you have high blood sugar or hypothyroidism affecting the nerve endings. The other possibility is an internal ear problem. This is quite possible as you have tinnitus. If possible go for a tilt table test. It an also be due to pinched cervical spinal nerve as you have had spine surgery in past. Many a times no imaging shows any pressure on nerves. However the symptoms of dizziness and unexplained falls do occur. Physiotherapy often helps prevent such falls. Brain injuries tumors transient ischemic attacks and bleed in the brain have probably been ruled out in your case. It can also be a vasovagal attack. Hypoglycemia severe anemia and vasovagal attacks are common causes of such falls. So blood sugar and blood pressure monitoring may be required round the clock. It can also be MS but other causes need to be ruled out first. It can also be a sleep related disorder with extreme sleep deprivation. So a sleep study should be done.
